HSC § 11362.735 Effective Jan 1, 2004Div. 10 · Ch. 6 · Art. 2.5

(a)An identification card issued by the county health department shall be serially numbered and shall contain all of the following:

(1)A unique user identification number of the cardholder.

(2)The date of expiration of the identification card.
