The Court notes the following deficiencies with the filings, to be addressed at the hearing: 1. Petition (Form GC-310) does not indicate whether proposed conservatee is receiving nor entitled to receive benefits from the VA- Item # 4(b).
2. Item # 16 on Form GC-314 (Conservator Screening Form) has not been completed.
The Court intends to continue this hearing for statutorily sufficient service of notice and filing of the Regional Center report.
