The petition fails to attach Petitioner’s proposed hourly fee schedule. (Prob. Code, § 1821, subd. (c)(1).)

Subject to a verified supplement that provides the following, the Court intends to grant the petition for a conservatorship of the person and estate except as follows.

The major neurocognitive disorder (“MND”) attachment to the capacity declaration fails to complete ¶ 22.d regarding whether a secured placement is or is not the least restrictive placement for the proposed conservatee. At ¶ 23.b, the attachment also fails to describe any MND medications needed and the potential benefit of each medication. Accordingly, the declaration fails to support all of the required findings set forth in Probate Code section 2356.5, subdivisions (b) and (c), by clear and convincing evidence. Therefore, no MND powers will be granted at this time. The Court will continue the matter if Petitioner seeks to submit a more complete declaration.
